In gcctonermanagement upload page I want this input fields 
company name has text field,  printer model name in dropdown get from api, after that selected model load the toner model  onchange dropdown, 
and then qty in number field
and DC upload options that two dropdown will come under printer_model_list and toner_model_list in toner model table I mapped pid that is printer model list id so based on printer selection onchange model dropdown and save details in this table
